If you are covered by a Costco employee insurance plan, please make sure to provide us with your insurance card and Costco employee number. If you are not a Costco employee, we would be more than happy to provide you with an itemized invoice with the Billing and Diagnostic Codes to submit to your insurance for reimbursement.
